Gliomas da ínsula: revisão anatômica e sua correlação com a extensão da ressecção, morbidade e sobrevida

Resumo
Introdução: A ínsula apresenta anatomia complexa. A escolha de corredores transsilviano ou transcortical para ressecção de gliomas insulares permanece controversa, e as principais preocupações são a lesão vascular durante a dissecção transilviana e o comprometimento funcional no acesso transcortical.
Objetivo: Revisar a anatomia da ínsula e comparar se há diferença entre a extensão da ressecção, a morbimortalidade pós-operatória e a sobrevida entre as 2 abordagens.
Método: Revisão feita colhendo informações publicadas em plataformas virtuais em português e inglês. Inicialmente foi realizada busca por descritores relacionados ao tema “gliomas da ínsula, mapeamento cerebral, acesso transsilviano, acesso transcortical, extensão da ressecção e técnica cirúrgica”, e seus equivalentes em inglês, com busca AND ou OR, considerando o título e/ou resumo. Após, foram incluídos somente os que tinham maior relação ao tema, e realizada a leitura da íntegra dos textos. Finalmente foram referidos 87 artigos.
Resultados: Não se evidenciou diferença entre as abordagens transsilviana e transcortical com relação à extensão da ressecção, morbidade pós-operatória e sobrevida.
Conclusões: A discussão entre as diferentes escolas de tipos de abordagem é inócua, sendo mais ideológica, visto que refletem a rotina de diferentes grupos de neurocirurgiões e não é baseada em argumentos estatísticos.
